| To register for the 2008-2009 Victory Academy Lions Evaluations, please print and fill out the following forms. All forms along with your evaluation fee can be turned in by mail or in person at the gym. Get $5 OFF your evaluation fee when you register by April 12th!! Lions Evaluation Information Sheet All new and returning team members must turn this form in. Victory Academy Registration/Insurance Waiver All new Victory Academy members or any returning member with a change in information must turn this form in. 2008-2009 Lions Handbook Agreement Form Please read the handbook in its entirety and then initial and sign the last page, which is the Handbook Agreement Form and turn it in. Evaluation Fees New Members: $10/person by April 12th - $15/person after April 12th Returning Members: $5/person by April 12th - $10/person after April 12th Beat the rush on registration day and save money by registering early!! Also, if you are a returning member and you refer a new member, you will receive $15 off of your June invoice, and $5 off for every additional person up to $30 off! |
