| To register for the 2011-2012 Victory Academy Lions Evaluations, please print and fill out the following forms. All forms along with your evaluation fee and a copy of your child's birth certificate can be turned in by mail or in person at the gym. Get $5 OFF your evaluation fee when you register by April 7th!! Lions Evaluation Information Sheet All new and returning team members must turn this form in. Victory Academy Registration/Insurance Waiver All new Victory Academy members or any returning member with a change in information must turn this form in. 2011-2012 Lions Handbook Agreement Form Please read the handbook in its entirety and then initial and sign the last page, which is the Handbook Agreement Form and turn it in. Payment Option and Drop From Squad Policy Agreement Form This form outlines our payment policies and must be signed and turned it. Credit or Debit Card Authorization Agreement Form It is optional, but strongly encouraged that each family enroll in auto billing for monthly tuition payments. If you choose to use this method, please complete this form and turn it in.
